What to Expect at Your First Visit

When you come to our dental office for your first appointment, we complete a thorough examination and talk to you about any recommended treatments. Then, you have the opportunity to ask questions and learn about your options before deciding on services for the future.

Prepare for your first appointment by bringing this information to your examination:

  • Dental insurance information (if you have dental coverage)
  • List of current medications you are using, such as aspirin, heart medications, anticoagulants, etc.
  • Any digital imaging taken by a previous dentist, if applicable

Also, talk to our team in advance if you have any medical conditions, such as high blood pressure, diabetes, artificial joints or heart valves, rheumatic fever, etc.

Please Note: Patients under the age of 18 must have a parent or guardian accompany them to the appointment.

Patient Forms

Below are the patient forms required for all new patients. To expedite the check in process, we recommend printing and completing these forms to bring with you to your first appointment. Should you have any issues with completing the forms, our friendly staff are standing by and willing to help.
